Physical Therapist – Full-Time, On-Site

We’re seeking a dedicated and compassionate Physical Therapist to join our team in supporting older adults through personalized, goal-driven care. In this role, you’ll help participants regain independence, improve mobility, and thrive in their daily lives—all while educating and empowering their families for continued success at home.

What You’ll Do:

• Collaborate with the interdisciplinary care team (IDT) to conduct assessments and develop individualized care plans
• Evaluate participant strength, balance, and physical function to build effective therapy strategies
• Provide one-on-one and group therapy sessions focused on recovery, mobility improvement, and functional maintenance
• Implement treatment plans that align with physician directives and care team goals
• Deliver therapeutic exercises, gait and mobility training, and support with assistive devices
• Track progress through detailed documentation and recommend adjustments as needed
• Educate participants and family members on therapy techniques and strategies to encourage ongoing progress at home
• Identify needs for additional services and coordinate referrals when appropriate
• Maintain and manage therapy equipment to ensure safety and readiness
• Collaborate with staff across skilled nursing, assisted living, and home-based environments to support participant rehabilitation needs
• Occasionally conduct in-home visits or on-site therapy at skilled nursing facilities (SNFs)

Schedule & Work Environment:

• Monday to Friday, 8:00 AM – 5:00 PM
• 100% on-site position with occasional travel to participant homes or partner facilities as needed
