Occupational Therapist – Home Health

Location: Local Travel Required | Flexible Schedule

Position Overview:

We are seeking a compassionate and skilled Occupational Therapist to deliver in-home therapy services to patients under physician orders. This role includes evaluating patient needs, developing personalized care plans, implementing therapy programs, and collaborating with both the attending physician and the care team to promote patient independence and improve physical and cognitive function.

Key Responsibilities:

• Conduct comprehensive assessments to evaluate patient capabilities and establish individualized therapy goals.
• Develop and implement treatment plans tailored to restore or enhance daily living skills, pending physician approval.
• Educate patients and family members on therapeutic techniques, adaptive tools, and self-care strategies.
• Train patients and caregivers on the safe use of orthotic, prosthetic, and assistive devices; fabricate or modify devices as needed.
• Perform regular functional and cognitive reassessments to adjust therapy plans when necessary.
• Provide consistent communication and updates to the attending physician regarding patient progress and treatment adjustments.
• Maintain detailed and timely clinical documentation in accordance with agency standards.
• Refer patients to outpatient occupational therapy services when appropriate and necessary.
• Offer consultation and guidance to other home health team members on therapeutic techniques and equipment use.
• Prepare reports for physicians and third-party payers documenting care provided and patient outcomes.
• Stay current with professional best practices through continuing education and in-service training.
• Actively participate in staff meetings, case conferences, and other agency development activities.
• Fulfill other related duties as assigned by the supervisor.

Schedule and Shift Details:

• Flexible scheduling based on patient needs.
• No on-call unless specifically indicated otherwise.

Travel Requirements:

• Local travel to patient homes within the service area.

Qualifications:

• Licensed Occupational Therapist (OT) in IL
• Home health experience preferred but not required.
• Strong interpersonal skills with the ability to educate and support patients and families.
• Reliable transportation and willingness to travel locally.
